How can I get a copy of my medical records?

To obtain copies of your medical records, download and complete a Request for Access to or Release of Protected Health Information form and bring it to the Health Information Management Department at Memorial Hospital during regular business hours, Monday through Friday 8 AM to 5 PM.  You may also submit your request form by mail, email, or fax to:

How can I get a copy of my child's birth certificate?

All birth certificates are issued by the Bureau of Vital Statistics. While Memorial Hospital does not issue birth certificates, our staff will assist new mothers with completing and submitting the necessary information to the Bureau of Vital Statistics.

To obtain a copy of your Nebraska certificate of birth, you must contact the Department of Health and Human Services or call 402-471-2871.

Can I obtain my lab results by phone?

Your physician can release lab results over the phone if the results are being made available as part of a visit or treatment episode.  All other requests for lab records should be directed to the Health Information Management Department at 402-694-8284. 

Please note that you may access your medical records, obtain test results, message your provider, and more through our Patient Portal.
