Billing Guide

The following information is designed to help simplify the complex world of healthcare billing. If you have any questions regarding Memorial Community Health’s billing processes, please contact us at 877-810-8050 or 402-694-8208. Our hours of operation are Monday through Friday 8:30 AM to 4:00 PM.

First Appointment

When arriving for services, please have your insurance card(s) with you (if you have current insurance coverage). The following information pertaining to your insurance policy is necessary in order to bill your insurance company for services rendered:

  • Insurer name
  • Insurer mailing address
  • Policy group number
  • Policy identification number
  • Policy holder data (name, address, birth date, employer, etc.)


Other items that you should bring to your first appointment include the following:

  • Referral form (if required by your insurance carrier)
  • Advance Directive (living will and/or durable power of attorney for healthcare, if you have one)
  • Driver’s license or other form of photo identification to confirm your identity.

Pre-Authorization / Pre-Certification

Many insurance plans have pre-authorization or pre-certification requirements. (Pre-authorization and pre-certification refer to the process by which a patient is preapproved for coverage of specific medical procedures and/or prescription drugs.) As the patient it is your responsibility to review your insurance plan to clarify specific benefits and requirements prior to any appointment or procedure in order to avoid non-payment or reduction of payments by your insurance carrier.

Medicare

Memorial Community Health will bill Medicare for services rendered and will also submit a bill to your supplemental or private insurance carriers as applicable. As the patient, you are responsible for any deductibles, co-payments, or other amounts not covered by Medicare.

Medicaid

All Medicaid patients are required to present a current eligibility card at the time of patient registration. Patients who fail to verify coverage will be responsible for payment. Once verified, Memorial Community Health will bill Medicaid for services rendered. As the patient, you are responsible for any deductibles, co-payments, or other amounts not covered by Medicaid.

Workers Compensation

Memorial Community Health will bill your employer/carrier for treatment of any work-related injury for which the employer/carrier accepts responsibility. As the patient, you and/or your insurance company will be responsible for any bill that the employer/carrier denies.

Third Party Liability

Memorial Community Health will bill the carrier of a third party for treatment of any injury for which the third party was deemed liable. If you are a Medicare or Medicaid recipient, health care laws require that we bill the liability carrier first, then Medicare or Medicaid. As the patient, you and/or your insurance company will be responsible for any bill that the third party/liability provider denies. Please note, if you are insured by Blue Cross Blue Shield, they require us to bill them first for liability.

If You Do Not Have Insurance

Memorial Community Health will extend a four percent discount to uninsured individuals who make payment within thirty days of billing. We accept checks, money orders, and approved credit cards. If you are unable to pay your bill in full within thirty days, Memorial Community Health is willing to explore a monthly payment plan option. Individuals who do not meet payment plan guidelines may be eligible for Memorial Community Health’s unsecured bank loan program and/or state and/or Memorial Community Health financial assistance. Financial Assistance Program

Memorial Community Health offers qualified patients assistance designed to provide payment discounts up to the full amount of charges. Guidelines issued by the Housing and Urban Development Department are used to determine patient eligibility. A completed application and proof of income are necessary for consideration.
