Home Health

Memorial Hospital works closely with various area home health agencies to provide skilled health services to patients at their place of residence. Memorial Hospital will work with you, the patient, to determine which service provider best meets your individual health care needs.

To qualify for home health services, you must be homebound and require at least intermittent skilled nursing care, physical therapy or speech/language therapy. You must also live within the licensed area of the home health provider and the care must be ordered by a licensed physician.

List of Services:

Skilled Nursing

  • Supervision and administration of medications
  • Intravenous therapy / TPN
  • Treatments (catheter change, sterile dressing changes, wound care, etc.)
  • Patient, family, and caretaker education
  • Diabetic management counseling
  • Nutritional assessments
  • Lab services( in some cases)
  • Observation and evaluation of unstable medical and surgical condition
  • Post operative assessment

Occupational and Speech Therapy

  • Rehabilitative program
  • Swallowing disorders
  • Rehabilitation for neurological diseases such as Parkinson's and MS
  • Caregiver education
  • Home Safety Assessment

Physical Therapy

  • Rehabilitative program
