At Memorial Community Health, our pharmacy department is a vital part of the healthcare team. Our pharmacists are dedicated to ensuring safe, effective, and personalized medication use for every patient. The pharmacy team plays an essential role in treatment to ensure patients receive the right medications and that they are given at the right time in the right way.
Clinical services include renal drug monitoring, drug interaction monitoring, therapeutic drug monitoring, pharmacokinetic dosing and medication order review. Memorial Hospital pharmacists also assist in medication reconciliation to help facilitate seamless transitions of care. This comprehensive review helps to avoid medication errors such as omissions, duplications, dosing errors, and drug interactions.
The pharmacy department also leads our Antimicrobial Stewardship Program to ensure antibiotics are used responsibly. Overall goals of the program are to reduce antibiotic overuse and antimicrobial resistance.
To ensure continuous care, we provide 24/7 pharmacy support, including remote pharmacist coverage after hours. On-site operating hours are Monday to Friday from 7 am to 5 pm. Memorial Hospital’s in-house pharmacy is staffed by two registered pharmacists and two pharmacy technicians.
