Notice of Data Security Incident

Memorial Community Health, Inc. (“Memorial”) has become aware of a data security incident that may have resulted in the advertent exposure of limited health care information. Although at this time there is no evidence of sensitive information being misused, Memorial has taken steps to send letters to anyone whose information may have been exposed. The F.B.I. also has been notified as have appropriate regulators.

On April 6, 2022, Memorial detected and stopped a network security incident resulting in unauthorized access to a limited number of individuals’ health care information. Fortunately, our robust technical safeguards in place quarantined the threat before a more invasive attack could occur. 

Upon learning of the network security incident, Memorial immediately engaged specialized third-party forensic and technical resources to respond to the incident and determine if any information was impacted. Memorial reviewed all network locations where sensitive data could have been viewed, and then arranged for notification of all individuals whose sensitive data could have been exposed.

Memorial remains committed to protecting sensitive data. The company has taken steps to send letters to known persons whose information may have been exposed. These letters include additional information about what occurred, credit monitoring services being offered, contact information for Memorial, and a toll-free number where individuals can learn more about the incident. The call center is available Monday through Friday (excluding holidays) between 7:00 AM to 7:00 PM Central Time at 800-405-6108.

The privacy and security of the sensitive information that we maintain is a top priority for Memorial, which deeply regrets any inconvenience or concern this incident may cause.
