The Connected Care Program is provided by Memorial Health Clinic and is designed to help you navigate your journey of managing multiple health conditions.
Our on-site care coordinator is specially-trained to work with you and your healthcare team in monitoring your health and taking the steps needed to reach your individual healthcare goals.
How can our Connected Care Program help you?
Managing various conditions, such as high blood pressure, diabetes, COPD, heart failure, arthritis and other conditions can be made easier with ongoing medical attention.
Through our connected care program, our care coordinator will help create an individualized treatment plan tailored just for you. This plan includes setting strategies to help you manage your health and includes information on current treatments, staying on schedule, and establishing reasonable goals.

- Become involved in coordinating your treatment program
- Stay in touch to help assure your needs are being met
